Two easy steps to have a superb learning experience:
- Reserve a space. Send the form below to the registrar with your deposit.
- To join a Certification group, send the Application below to the registrar.
The registrar will send a co-verification form to your Advanced Practicum Supervisor which she or he will return to the registrar with a copy to you. So, please be in touch with your Advanced Practicum Supervisor.
About mailing, emailing and paying.
If you wish to pay by cheque or money order, please mail us your forms with the payment.
If you wish to pay by credit card, please scroll down to the PayPal button.
(Please note that there is a transaction fee for using a credit card.)
You can then email your forms to the registrar.

There is an administration fee when using PayPal.
- Forms in Word format are downloaded to the Downloads Folder on your computer. They can then be filled in, saved for your own information and sent as an email attachment to the registrar of the certification training. The email address is included in the form.
- Forms in PDF format must be saved first. Please use the latest Reader, Adobe Reader X. http://get.adobe.com/reader/otherversions/ or later.
- A tool bar appears when you bring your mouse arrow to the bottom right hand corner of the document. Click to save the document. You can then enter your information, save it and send it as an attachment to the registrar’s mailing address on the form or as an attachment to an email.
- Be patient for the software to process the form. You can of course also mail your forms.
